Used if you wish to remove your AWAI account. Click on “Submit”. Before submitting, you will be given the possibility of adding some comment if you wish. A document will be generated; you may click on "Download document" (the form will also be sent to you via email) in case you need it for your records.

A confirmation message will be sent after execution via email.

Please note that this action is final. Once executed if you should need access to AWAI you will have to request it from the beginning as a new user.
